Should I book an appointment?
If you are 18 years or older and experiencing cognitive difficulties, anxiety, depression, or psychosis, consulting with Dr. Hugo may be helpful. Our clinic specialises in treating common psychiatric conditions such as anxiety, depression, and psychosis, as well as neuropsychiatric conditions caused by medical issues affecting the brain, including head injuries, stroke, and dementia such as Alzheimer’s disease.
If you’re unsure whether an appointment is necessary, consult with your GP.

More information on your first appointment
Your first appointment will last for an hour. There will be out-of-pocket expenses. Please discuss expenses and payment options when you book your appointment.
Before your first visit, please fill out either the 20-minute or 5-minute questionnaire. The 20-minute questionnaire will provide me with more information to better assist you, but if you’re pressed for time, you can opt for the shorter 5-minute version. Simply click the link below to download the questionnaire of your choice.
To ensure we have enough time to complete the necessary forms, please arrive 20 minutes before your scheduled appointment time.
During your first appointment, we will conduct a thorough psychiatric assessment. We appreciate your honest answers to our questions. If you prefer, you can take notes beforehand and share them during the consultation. Additionally, please bring a list of your current medications, or even bring the medications with you.
We encourage you to bring someone who knows you well, as they may be able to provide additional information.
Rest assured, all information will be kept confidential, and with your permission, a report will be provided to your GP.
Will there be further appointments?
Appointments are available in 15-minute, 30-minute, or hour-long sessions. Out of pocket expenses apply, and you should discuss them when booking your next appointment.
Follow-up appointments:
It is essential to provide comprehensive information to help streamline follow-up appointments. Furnishing additional information can enhance decision-making for your clinician.
A Treatment Update Form (click here) is a document used to capture relevant updates about your condition and progress before a treatment session.
A Mood Chart (click here) is a tool for tracking and visualising mood over time. It typically involves recording daily mood ratings, ranging from very depressed to very elevated, alongside other relevant factors such as sleep patterns, medication, significant life events, and physical activity.
The Sydney Bipolar Screener (SBS) (click here) is a diagnostic tool designed to help identify individuals who may have bipolar disorder. It consists of a series of questions that assess mood symptoms and fluctuations in mood over time. The screener aims to differentiate bipolar disorder from other mood disorders, such as depression, by focusing on the episodic nature of mood changes characteristic of bipolar disorder.
The Adult ADHD Self-Report Scale (ASRS-v1.1) (click here) Symptom Checklist is a tool used to screen for symptoms of Attention Deficit Hyperactivity Disorder (ADHD) in adults. Developed by the World Health Organization (WHO) in collaboration with researchers and clinicians, the ASRS-v1.1 consists of 18 questions designed to reflect the DSM-IV criteria for ADHD.
